I do have one I bought but I've actually never used it, it just sits on my mantle. The bottom one was done using a Quaker Oats tin container. The top one was the school's camera so I would assume it was purchased.

i am making one right now, i need some flat black paint to finish it. but its super easy to do

you need a light tight box or something like that, light sensitive material, you can get film just for it, but i use printing paper and you make a hole the size of a pin and you put your light sensitive material at the back and when your ready to expose you uncover the pinhole.

theres guides online for how long you should expose for certain lights.

yep. they also make premade boxes for them, nice one will run you about $100. They're kinda cool and are usually very low ISO and very WIDE angled. You can use a specific Polaroid film for it or negatives for the film. You can even make them out of different everyday things (like a tin can as long as it has a lid).
